Did you know that nowadays it is actually considered unethical to do MS medication trials with a placebo/untreated group? Most new trials compare the test medication with one of the more mild medications like interferon. This is because we have plenty of data on the natural history of MS - a.k.a. what happens if you don’t treat (at least on a population level) and it’s not good! Compared to 30 years ago, people living with MS today are less disabled, have less relapses, and now have a normal life expectancy. We still have a lot to learn about progressive disease, but it seems that hitting the immune system hard early in the relapsing course really changes the trajectory for many people.
